WHPS PERMISSION FORM

PARENTS NIGHT OFF - PLEASE READ

By Checking the box for "I Agree to the WHPS Permission Form" on the Registration Page, you are agreeing to the following:

     My child(ren), listed on the Registration Form, have my permission to participate in the Parents at Woodland Hills Private School (PAWS) Parents Night Out on September 27, 2019.

     It is hereby agreed that all persons participating in this event and their parents have waived all claims they might have against PAWS or WHPS and their respective employees and volunteers for any injury or accident which occurs during or by reason of the event, except only those due to violation of the law.

     Shall it be necessary for my child to have medical care while participating in the event, I hereby give Woodland Hills Private School personnel permission to use their judgment in obtaining medical care for my child and I give permission to the selected medical personnel to render such medical care as seems necessary and appropriate.

     I agree to abide by the statement above.  I also agree to direct my child to cooperate with the directions and instructions of the Woodland Hills Private School staff in charge of the event.
